TRANSITIONAL YOUTH HOUSING

CCFR presently offers housing units as scattered sites, encompassing a total of 15 beds. TYHP grants young adults the opportunity to reside in a secure, unstaffed community setting for a duration of up to 24 months. The delivery of services within the Transitional Housing program will adopt a wraparound approach, enveloping individuals in support as they strive to diminish homelessness and attain self- sufficiency.

Any Young Adult (ages 18-24
experiencing homelessness
or at risk of homelessness
can be referred to the TYHP
by filling out a referral form. YAs can also call the CCFR Coordinated Entry line.

While funding is available!

The TYHP harnesses EOHHS funding to empower young adults in transcending financial obstacles, including housing instability, pursuit of secondary education, settling arrears, transportation needs, and the journey toward self-sufficiency.

With a heart for the community, the TYHP features a street outreach initiative, deploying a well- equipped RV to meet young adults in their environments, offering essential resources and fulfilling basic needs.

YOUth Centered; YOUth led!

Bristol County’s Youth Advisory Board (YAB) is a welcoming space for young adult who have faced the trials of homelessness. Here, they come together as a lively community to support each other and advocate for their peers. The YAB amplifies their voices, ensuring they resonate at the state level. The YAB gathers real, meaningful feedback that helps improve the services and systems designed to support them. Together, the members are empowered to create their own strategies, missions, values, and goals.
