The Coordinated Entry System is the central referral system for all people experiencing homelessness who are accessing Emergency Solutions Grant (ESG) and Continuum of Care (CoC) programs, funded through the Department of Housing and Urban Development (HUD). These programs consist of Homeless Prevention, Rapid Re-Housing, and Permanent Supportive Housing, which serve households both at-risk of homelessness, and those who are currently experiencing literal and chronic homelessness.

This system was designed to help both individuals and families resolve their housing crisis by accessing valuable resources in an equitable, person-centered, and transparent manner. The system serves as a centralized way to triage and refer those fleeing domestic or gender-based violence, those at-risk of homelessness, and those currently experiencing literal and/or chronic homelessness. The aim of the system is to prevent homelessness and/or provide permanent housing for those currently experiencing homelessness. Households who call are assessed for eligibility and the most appropriate intervention. This assessment is used to prioritize the most vulnerable for scarce resources.
