GENERAL CORI FORM REQUIREMENTS

Eligibility

  • All persons age 17 and over must complete the CORI form and all other Diocesan requirements AND must be approved by the Office for Child Protection before working with minors.
  • Individuals who are under the age of 17 may have a CORI done if appropriate for his/her role and with parental permission.
  • Priests, deacons and religious must complete forms through the Vicar General’s office.  CSS does not handle CORIs for clergy and religious.

Form requirements

  • Must be on the correct form, currently the one issued June 2015;
  • Must be on the correct color of paper: blue for volunteers, yellow for employees, orange for subcontractors;
  • Must be completed in INK only; no pencil allowed;
  • No white out allowed.  Mistakes should be crossed through with one line and initialed;
  • Applicant must complete ID information, such as state of issue and ID number;
  • Authorized verifying employee must print type of ID where indicated, print name, and sign and date form where indicated (page 2); and
  • Original CORI form must be submitted to the CORI Department; no faxes or copies of CORIs can be accepted.

Additional forms

  • Front copy of photo ID must be attached;
  • The Code of Conduct must be filled out completely;
  • If the applicant is a volunteer submitting a CORI for the first time only, the Volunteer Questionnaire must be attached; and
  • If the applicant has a Rhode Island address or ID, the RI Disclaimer must be attached and notarized prior to submitting the forms to CSS.

SUBMISSION REQUIREMENTS

  • Submit promptly!  All training paperwork and/or CORI renewals must be submitted no later than the end of each month.  Trainers who hold CORI forms for months are out of compliance with The Charter, since anyone who even may have unmonitored access to children must have a current, clear CORI, a valid training date, and all required paperwork.  Only people cleared by the Office for Child Protection may be working with the children of our Diocese!
  • Use the current training sign-in sheet, dated June 2014.  Remember to initial in the “T.I.” column on the far left of the form.  This is an audit measure to verify that the names on the form are people who actually attended the training.
  • All sets of CORI paperwork for each individual must be stapled together and put in alphabetical order by last name before sending them to the CORI Department.
  • Order.  The correct order for each person’s paperwork is as follows:
    • CORI form, side 2 facing up (easier to read printed names)
    • Code of Conduct, side 2 facing up
    • Volunteer questionnaire (if needed; side 2 facing up)
    • RI Disclaimer (if needed)
    • Photo ID

      Once all sets of paperwork are in alphabetical order, paper clip the training sign-in sheet over the entire set (if it was a training session) and send it to the CORI Department.
